There will be a Flu Shot Clinic in the basement of Security National Bank in Orange City Wednesday, October 8th, from 7:30 a.m. to 9:30 a.m.
A consent form will need to be filled out before you attend. That form is here.
You will also need a photo copy of your insurance card.
Orange City Area Health System will be bringing the Trivalent vaccine, which is recommended through age 64.
There is also a High Dose vaccine that is recommended for ages 65 and older.
If any your employees are 65 or older, and in need of the High Dose vaccine, they are available at Orange City Area Health System Medical Clinic 737-2000.
